In office appointments should be booked for any medical concern that is physical
(e.g., body pain, nausea etc.)

Virtual (Video)
Virtual Video Appointments are conducted via Microsoft Teams.
Due to new ministry guidelines, virtual appointments are available to patients that are rostered or have been seen in office by the provider in the last 24 months.

Counselling
Mental health counselling appointments can be conducted in person or virtually.

Annual Assessments
Annual assessment are carried out every year for each patients. Prior to each annual assessment appointment, you will receive an online questionnaire that must be completed in advance.
How to prepare for your appointment:
- fill out questionnaire
- fast for 8-12 hours (can take medication and drink water)
- you may be required to provide a urine sample
